School Responsibilities

School Process for Issuing the Partial Waiver Form to students:

  1. The principal or principal’s designee shall issue the age appropriate (either for the 14 and 15 or 16 and 17 year – old students) partial waiver form by signing his/her name on the form where it indicates Superintendent or Designee. Note: The other signatures indicated on the form may be added but are not required.
     
  2. When using the waiver form for 16 and 17 – y ear old students, the responsible party for signing the waiver form should delete the items he/she are not approving, print the form and then initial the items they are approving.
     
  3. Inform students that the partial waiver form given to the employer is only good for one (1) year from the date it is signed.

All Other Minors:

Minors NOT enrolled in (K-12) public schools: drop outs, students who are home schooled, in private school, enrolled in an alternative education program (GED), or have been expelled may apply for a partial waiver from the Child Labor Compliance office by calling 1-800-226-2536.

Upon request, students registered with the District home education program office shall be given an Acknowledgement of Home School Enrollment document to submit with their partial waiver request application to the Child Labor Compliance office.
